Will filler in my smile lines look natural,
or just look filled?

Filler in the smile lines looks natural when the product, depth, and placement are chosen for your face, the first treatment is conservative, and the settled result is checked before anything is added. It looks filled when the crease is over-volumized on day one. Here is how to tell the two plans apart at any practice, and how we plan Evolysse Form at Reviv.

Why it is a fair worry

A softened fold,
or a filled one.

Among the low-rated reviews of medical spas within thirty miles of Millbrae, a result that looked overdone or uneven instead of natural is a complaint that recurs at practice after practice: it came up at eighteen different practices in our most recent read of the regional record. It is not the most frequent complaint on the Peninsula, but it is the one people can see across a room, and the smile lines are where it shows first, because the fold sits beside the mouth, where any change is noticed.

Fillers add volume to areas that have lost it, and the "filler face" look comes from over-volumization or chasing trends rather than respecting individual proportion. A nasolabial fold that is over-volumized on the first visit is exactly where that look begins. Filler is precision work: product selection, depth, and placement matter more than syringe count, and a fold treated as a syringe count rarely reads as a softened fold.

Our own post from the studio, filmed during the treatment in the photograph above, puts the goal plainly: "Beautiful aesthetic treatments are about enhancing, not changing what makes you unique," and Evolysse Form is there "to soften the appearance of nasolabial folds while maintaining a natural look and facial expression." That second phrase is the standard this article is about. A softened fold still looks like your face. A filled one looks like the filler.

What to look for anywhere

Four things to ask
before filler goes near a smile line.

Use these at any practice, including ours. The answers tell you whether you are getting a plan or a syringe count.

  • Which product, and why this one. A deeper fold and a fine surface line are different jobs; a good injector names the gel for each and can say why. Ask whether it is a hyaluronic-acid gel, which can be dissolved if it ever needs to be, and what the plan would be if it did.
  • Where it goes, not just how much. Depth and placement matter more than syringe count. Ask where the product will be placed relative to the fold and what the injector is doing to keep the result in proportion with the rest of your face.
  • A conservative first visit with a scheduled look. Ask how much will be placed on day one, when the settled result will be assessed, and whether adding more later is part of the plan rather than an upsell.
  • A named, licensed injector and a physician behind the plan. Ask who will hold the syringe, look up the license, and ask which physician reviewed the plan. Then ask for the price in writing before you sit down.
How Reviv does it

Form for the fold, Smooth for the surface,
and a look at the settled result before anything is added.

At Reviv Med Spa & Laser Treatment in Millbrae, Evolysse is placed by Dr. Gayle Misle, MD, an Anesthesiologist holding CA Medical License G38840 and our Founder and Medical Director, and by Megan Sosnick, MSEL, MSN, RN, PHN, CNL, our Practice Manager and Aesthetic Nurse. That is Megan in the photograph above, working along a patient's smile line in our studio on El Camino Real, a few minutes from Burlingame and San Bruno. You choose the provider you want to see when you request an appointment and choose the provider you would like to see, and both are among the four people who treat you, listed with their licenses.

We carry both gels in the line. Evolysse Form is the more robust gel for deeper folds, like nasolabial folds, and areas needing structure; Evolysse Smooth is the softer gel for fine lines and surface refinement. Many treatment plans use each where it belongs: Form to rebuild the fold, Smooth to finish the surface. At your consultation we map which product belongs where, and whether Evolysse or another family on the hyaluronic-acid filler menu we place conservatively is the right tool for each area. Expression lines that show only in motion are a neurotoxin conversation, and we will say so.

Then we treat conservatively. We map your anatomy before we treat, choose the right depth and plane for each product, and inject conservatively with vascular safety front of mind; we would rather under-fill at the first appointment and add at your two-week touch-up than over-correct on day one. Volume is visible immediately, swelling and any bruising peak over days one to three, and the gel integrates and settles into its final placement over days four to fourteen. On day fourteen you come back for a complimentary touch-up assessment, where we evaluate the settled result and fine-tune placement if needed. That appointment is the difference between a plan and a syringe count: the decision to add is made looking at your settled face, not at a mirror on treatment day. And because both Evolysse gels are hyaluronic acid, they can be dissolved with hyaluronidase if ever needed, the same safety valve as other HA fillers; the better practice is precision from the start.

Pricing is per syringe and quoted in writing at consultation: Evolysse starts at $850 per syringe, most first treatments use one or two syringes depending on the area and the correction needed, and there are no surprise charges and no day-of upsells. If a non-reversible structural filler is the better tool for your face, we carry that too, and we will tell you plainly why.

After eighteen years in Millbrae, with more than 300 reviews on Yelp and about 150 on our Google Business Profile, the line we would show a wary patient from Hillsborough or San Mateo is one a patient of more than five years left on Google: Dr. Misle "has helped me approach aesthetics in a natural way that enhances my features without looking overdone." That is the standard for a smile line here: softened, and still yours.

Questions

Asked at the front desk,
answered here.

Does Evolysse Form look natural in the smile lines?

It looks natural when the gel is matched to the job and placed conservatively. Evolysse Form is the more robust gel for deeper folds like nasolabial folds; Smooth is the softer gel for fine lines, and many plans use Form to rebuild the fold and Smooth to finish the surface. We treat conservatively and add at the two-week touch-up assessment rather than over-correct on day one. Evolysse Smooth and Form, from $850 per syringe has the full timeline and pricing.

What if my smile line filler looks overdone or uneven?

First, give it time: swelling and any bruising peak over days one to three and the gel settles over days four to fourteen, so the contour is judged at the day-fourteen assessment, not on day two. If the settled result is uneven, placement is fine-tuned then. Both Evolysse gels are hyaluronic acid and can be dissolved with hyaluronidase if ever needed. Our day-by-day recovery notes for injectables and fillers cover the first two weeks, and if something looks wrong at any point, call us at (650) 697-3339.

How many syringes do smile lines need, and what does Evolysse cost?

Most first treatments use one or two syringes depending on the area and the correction needed. Evolysse starts at $850 per syringe, with Reviv membership pricing of $765 per syringe, and your exact plan is quoted in writing at consultation before any treatment begins. Why a written quote matters at a med spa explains how we handle pricing.
