Which body contouring treatment is right for you,
and how do you know you are not just being sold one?
It depends on what you are actually trying to change. Stubborn fat, cellulite, and loose skin are three different problems, and the three no-downtime devices we run in Millbrae each do a different job: cryolipolysis removes fat, radiofrequency tightens skin, and Coolwaves microwave energy reaches all three in one modality. The right one is the one matched to your tissue at consultation and written down before anything is booked, not the one on special this month. Here is how to tell the difference anywhere, and how we do it at Reviv.
A personalized plan,
or a package with your name on it?
Among the low-rated reviews of medical spas within thirty miles of Millbrae, one complaint has little to do with the treatment itself: feeling sold to, with packages and add-ons pushed during what was supposed to be a consultation. It came up at twenty-four of the fifty-five practices in our most recent read of the regional record, from San Francisco down the Peninsula, and body contouring is where it bites hardest. Three devices with overlapping names, each sold in cycles or a series, and a consultation that ends with a number before anyone has said which problem, fat, cellulite, or laxity, you actually have.
The reason it matters is that the devices are not interchangeable. Freezing fat does nothing for loose skin. Heating skin does not remove a fat pocket. A device that reaches both can still be the wrong choice if what you want is a smaller number on the scale, which none of them delivers. When the recommendation arrives before the diagnosis, the biggest package is the easiest thing to sell, and a series that treats the wrong problem produces a result you paid for and cannot see.
Our own post from the studio, introducing ONDA Pro, put the principle simply: "Every treatment plan is personalized because everyone’s goals are different," and "Our experienced team will help you determine whether ONDA PRO is the right option for you." A sentence like that is only worth something if the practice will also say, in writing, when it is not the right option. That is the test to apply anywhere, including here.
Four things to settle
before any body contouring series.
Use these at any practice, including ours. A provider who can answer all four without reaching for a brochure is recommending, not closing.
- A diagnosis before a device. Is it localized fat, cellulite, skin laxity, or some combination? Where does the fat sit, and is the skin over it still elastic? If nobody can tell you which problem you have, nobody can tell you which device you need.
- A reason for this device over the alternatives, including the ones the practice does not own. Cryolipolysis is purely a fat-reduction tool. Radiofrequency tightening is modest and gradual and does not remove fat. Microwave energy reaches the fat layer more selectively than superficial heating, which is why it can reduce fat as well as tighten. Ask what each device is cleared for, and why this one, on this area, for you.
- A result described as a contour change with a date on it. None of these treatments is weight loss. Ask what the change will look like, when it should be visible, what it will be compared against, and what happens if it is not there.
- A written plan with the session count and the price, that you can take home. A physician should have reviewed it, and leaving to think it over should cost you nothing, including follow-up sales calls.
Matched to your tissue,
written down, and no pitch.
At Reviv Med Spa & Laser Treatment on El Camino Real in Millbrae we run three non-surgical body contouring devices, and the honest description of each is on its own page. That is the console in the photograph above, the ONDA Pro in one of our treatment rooms, filmed by our team for the post that prompted this article. Coolwaves microwave contouring for localized fat, cellulite, and skin laxity in one modality uses patented 2.45 GHz microwaves that bypass the surface layers and are delivered mostly to the subcutaneous fat, where they disrupt fat-cell membranes while the same energy heats the dermis and prompts collagen to contract and rebuild. In more advanced cellulite the Coolwaves target the connective tissue between the fat lobules, the structure behind the dimpling. It carries U.S. clearance for non-invasive body contouring and the improvement of cellulite, it is planned as a series, generally 3 to 6 sessions of about 20 to 30 minutes spaced 3 to 4 weeks apart, and because the system is consumable-free there are no per-session applicator fees baked into the price, which is quoted in writing at consultation.
Cryolipolysis fat reduction planned in cycles and judged at twelve weeks is the other fat tool on our menu. CoolSculpting Elite freezes fat and is purely a fat-reduction tool: the cooling destroys fat cells, the lymphatic system clears them over eight to twelve weeks, and the literature reports up to about 25 percent fat reduction in the treated area per cycle. It is purely a fat-reduction tool, so loose skin and cellulite are not what it is for. Most areas need one or two cycles of 35 to 60 minutes, pricing starts at $750 per cycle, and the treated area may feel firm, tender, or numb for one to two weeks, which is a sensation to plan around even though there are no restrictions on activity. We are an Allergan Platinum Plus 2023 partner, and we still say plainly when it is the wrong device for you.
Multi-frequency radiofrequency tightening for early-to-moderate laxity on the body is the third. NuEra Tight is modest and gradual: typical improvement is in the 10 to 20 percent range, most areas respond to a series of 4 to 6 sessions spaced 1 to 2 weeks apart, and the durable result is judged at month 3 to 4 against your pre-treatment baseline photographs. It has cellulite-relevant settings and shows modest improvement alongside tightening, but it does not eliminate cellulite, and significant excess skin needs surgery, which we say. Pricing starts at $350 per session.
Here is how the choice is made. At your consultation we assess where the fat sits, whether cellulite or skin laxity is part of the picture, and what a realistic result looks like for your body, and we recommend whichever device fits your anatomy and what you want to change. Dr. Gayle Misle, MD, an Anesthesiologist holding CA Medical License G38840 and our Founder and Medical Director, oversees the body-contouring program and reviews every ONDA Pro and CoolSculpting plan before treatment; Megan Sosnick, MSEL, MSN, RN, PHN, CNL, our Practice Manager and Aesthetic Nurse, maps your treatment areas, runs your sessions, and tracks your standardized progress photos through the series. Both are named with their credentials on the providers page, where every license is listed, and you can name the provider you would like to see when you request a consultation and pick your provider. Your plan and pricing are mapped in writing at consultation, before any treatment begins, we focus the plan on the areas that will actually benefit, and part of what our hours on these devices have taught us is which patients are better served by a different modality altogether. If the honest answer is that your goal is pounds rather than contour, the written plan says so and points you to medically supervised weight loss under a physician instead. Our consultations are no pressure and no obligation; how to get an honest opinion without the package pitch describes how they end, and the plan goes home with you.
After eighteen years in Millbrae, with more than 300 reviews on Yelp and about 150 on our Google Business Profile, our CoolSculpting and radiofrequency pages carry patient reviews of their own, and we say on the ONDA Pro page that treatment-specific reviews there are still building rather than borrowing praise from elsewhere. Reviv was the fifth med spa in the United States to take delivery of ONDA Pro and one of the first thirty practices nationwide to put it into regular use, and what that head start bought was hours on the device: which handpiece suits which depth, how much energy an area tolerates, and, just as often, when a patient from Burlingame, San Bruno, or Hillsborough is better served by cryolipolysis, by radiofrequency, or by neither. Once a series is chosen, how a tightening series is judged against a baseline photograph is the companion question.
Where this applies,
treatment by treatment.
Asked at the front desk,
answered here.
Should I choose ONDA Pro or CoolSculpting for a stubborn pocket of fat?
CoolSculpting freezes fat and is purely a fat-reduction tool; ONDA Pro uses microwave energy that warms the tissue, so in addition to reducing fat it also targets cellulite and tightens skin in the same treatment. If the pocket is fat alone and you are near a stable weight, cryolipolysis is the established choice; if laxity or cellulite is part of the picture, Coolwaves address all three. At your consultation we recommend whichever fits your anatomy and what you want to change, and a side-by-side comparison of duration, downtime, and results window is the tool for deciding at home.
Does no downtime mean I will feel nothing afterward?
No. None of the three requires anesthesia or a recovery period, and normal activity, including exercise, resumes the same day, but each has an expected after-feel. After ONDA Pro the area may feel mildly warm or look slightly pink briefly. After NuEra Tight the skin is mildly pink and warm for an hour or two. After CoolSculpting the treated area may feel firm, tender, swollen, numb, or tingly for one to two weeks, sometimes longer. We review the sensation timeline at consultation, and the recovery timeline, treatment by treatment sets it out in one place.
What if none of these is right for me?
Then that is what the written plan says. None of the three is a weight-loss tool, significant excess skin is surgical territory that radiofrequency will not match, and we screen for the conditions that rule each device out, including pregnancy, an implanted electronic device or metal implant in the treatment area, a hernia in the area, and the rare cold-sensitivity disorders that rule out cryolipolysis. If the honest recommendation is medically supervised weight loss first, or no treatment at all, you leave with that in writing, no obligation and no follow-up sales calls. Call us at (650) 697-3339 if you would rather talk it through first.
